Right after the revenge saga where Shank kills cartel leader Cesar to avenge his girlfriend's murder, he finds himself in another critical situation yet again. A ruthless militia has taken over town, led by the evil General Magnus. Little do the townsfolk know that Magnus is gravely ill and needs a heart transplant to survive. 

A rebellion is rising against General Magnus, however, his men kidnap the rebel leader Elena, who is Shank's mentor. They plan to kill her so that they may replace Magnus' heart with hers. Will Shank be able to go against Magnus and save Elena?
